This is the current, honest map of the remote-caregiving stack: what works, what still requires a plane ticket, and what the emotional cost of distance really looks like.
1. Teleconsultation Platforms
Practo, Apollo 24/7, MFine, and Tata 1mg all offer video consultations with Indian doctors. For routine follow-ups, prescription renewals, chronic disease management, and second opinions, these platforms are genuinely useful. Costs run INR 500-2,000 per consultation. NRIs can pay via international card.
What they don't replace: physical exams, imaging, in-person diagnostic evaluation. Emergency care still requires a nearby human.
2. ABDM / ABHA Health Records
The Ayushman Bharat Digital Mission's ABHA (health record) system now allows an NRI to access a parent's health records from abroad — with the parent's consent. Doctor visits, lab reports, and prescriptions increasingly get filed into the ABHA record. This means when you're on a video call with your parent's doctor, you can see the reports on your screen.
Practical setup:
- Create an ABHA ID for your parent (they need Aadhaar and a mobile number).
- Set consent access for you as the immediate family member.
- Encourage local hospitals and clinics to file records to ABHA (many now do this by default).
3. Wearables and Fall Detection
- Apple Watch (Series 6+): fall detection auto-calls emergency + configured emergency contacts.
- Samsung Watch (Watch5+): similar fall detection.
- Local India options: GOQii and Fitbit have India-facing subscriptions; medical-grade options like KardiaMobile for ECG.
Fall detection works only if worn. Many elderly parents resist wearing a watch daily. A patient conversation, done more than once, usually gets there.
4. Medication Management
The compliance problem — did they take today's dose — is the single biggest ongoing risk. Options:
- Physical weekly pill organiser filled by a local family member or attendant. Still the gold standard.
- Reminder apps (Medisafe, MyTherapy) on the parent's phone. Works if they respond to notifications.
- Smart pill dispensers (Hero, MedMinder) that alert you if a dose is missed. Best combination with attendants.
- Local pharmacy delivery services (PharmEasy, Netmeds, Apollo Pharmacy) with recurring refill setup.
5. Hiring and Vetting a Local Attendant
For semi-independent parents, a daytime attendant (4-8 hours/day) can bridge the gap between "fully independent" and "needs full-time care." Costs run INR 15,000-40,000/month depending on city and skills. Vetting steps:
- Reference-check with at least 2 prior employers by phone.
- Verify police-verification documentation (Passport Seva or thana-issued).
- Start with a 30-day trial before any long-term commitment.
- Set up a monthly video call review with your parent to check comfort and any concerns.
- Use an agency (Portea, HelpAge India, Emoha, Age Care India) for insured attendants — more expensive but reduces risk.
6. AI Translation for Doctor Calls
If you or your spouse do not speak the regional language your parent's doctor works in (Telugu, Tamil, Marathi, Bengali, etc.), real-time AI translation via Google Translate or Zoom's live-caption + translate feature makes joining a consultation viable. Not perfect, but usable for a follow-up call.
7. What Technology Cannot Solve
- Emergencies — someone has to physically arrive within minutes.
- Companionship — video calls help; they are not enough.
- Complex end-of-life care decisions — every family must have this conversation, in person, ideally before it's urgent.
- Grief and cognitive decline in real-time from a distance — the emotional cost is real and does not scale down.

- ABHA setup: free.
- Wearable + first year subscription: ~$300-$500 one-time + ~$100/year.
- Medication delivery: no premium over pharmacy price.

The Two Non-Negotiable Conversations
Set aside a real in-person visit for these two conversations, with your parents, with siblings if any:
- What are their wishes if a major health event happens — hospitalisation, surgery, life support.
- Who has power of attorney, where are the documents, and what accounts have you named on them.
Technology handles the day-to-day. These two conversations are what technology cannot.
